Preparing for Your Visit

Your first consultation with a RSNC intern will be approximately 60 minutes; return visits are approximately 60 minutes. You'll be asked to complete a detailed health questionnaire, designed to help you identify your health needs and provide your intern with a complete medical history.

First-time patients at the RSNC must complete several forms. In order to better facilitate your first visit, you're encouraged to download and complete the required forms beforehand and bring them to your first appointment. Completing the forms beforehand also allows you to take the time to review and include all of your prescription and medical history.
